The notary block is part of the form
Wyoming's form runs past the signature into a full jurat: state, county, the acknowledgment sentence, the notarial officer's signature and the commission expiry. Most states stop at the signature.

Three amounts, not one
The body of the Wyoming waiver asks separately for what has been received, the retainage being held back and the amount still unpaid. Those are three different numbers and filling them in carelessly is how a claimant releases retainage they meant to keep.
If retainage is still outstanding, it belongs in the retainage field, not the payment field.
The statute's own warning
Immediately above the form the statute prints a note to the lien claimant saying that signing this form has legal implications and that a claimant with questions about completing it should consult an attorney.

Does a Wyoming lien waiver need to be notarized?
The statutory form includes a notarial acknowledgment block, so in practice it is executed before a notary. That is unusual and it catches people who are used to other states.
Is there a separate Wyoming form for final payment?
No. One form covers every payment. The amounts you enter in the payment, retainage and unpaid fields are what fix how much is being released.
Can I use this form on a commercial job?
The statute prescribes one waiver form without splitting residential from commercial. As always, a job with real money at stake deserves a Wyoming attorney's eye on the completed document.
What happens in Wyoming if the payment is dishonored?
Read the form carefully, because it says two things in a row. It says the waiver may be relied on by the owner even if payment is accepted in uncertified funds and is later dishonored or revoked, and then it says the waiver shall not apply if payment tendered by the owner is dishonored or revoked. Those sentences pull against each other and this is one place where a Wyoming attorney is worth the call.
